Health Partners Primary Care Center is located on U.S. 301, North of La Plata in the Old Waldorf School across from Festival Way shopping center and next door to the Greater Waldorf Jaycees Community Center. A map and directions are provided below or you may use Yahoo or Mapquest.

Directions

From Cobb Island

Turn right on Neal Road. Neale Road becomes Neale Sound Road. Make slight right onto Cobb Island Road (Md-254). Turn left on Rock Point Road (Md-257) and proceed approximately 9.7 miles. Make right onto Crain Highway ((IS-301N) and proceed approximately 19.6 miles. Health Partners on right between Checkers Restaurant and the Waldorf Jaycees.

From Indian Head

Go north on Indian Head (MD-210N). Proceed approximately 8.9 miles. Make right turn on Md-228E (Berry Road). Proceed 6.6 miles. At main light in Waldorf on Route 301 make left turn. Health Partners is between Checkers Restaurant and the Waldorf Jaycees.

From Hughesville

From Route 231 make night at light onto Route 5 (Leonardtown Road). Proceed approximately 10.4 miles. At main light in Waldorf on Route 301 make right. Health Partners is between Checkers Restaurant and the Waldorf Jaycees.
